While major aviation accidents or incidents typically come with probable cause and safety recommendations, some events have resulted ininvestigatorsbeing unable to provide closure to those affected by the tragedies. However, those events are few and far between. While they do attract an unreasonable amount of conspiracy theories, some of the events have still resulted in safety improvements within the industry.
