A car has landed on train tracks at Waterfall, in Sydney’s south, after a three-vehicle crash on the Princes Hwy about 6.10am on Friday.
There are fears not all of the seven people who NSW Ambulance said were injured in the crash have survived.
Southbound lanes of the highway have been closed, causing traffic delays for commuters as emergency services rushed to the scene.
The impact of the crash sent one of the three cars involved through a fence before it landed on the railway tracks, NSW Police said.
Six men, between the ages of 18 to mid-30s, have been hospitalised.
Two of the men were taken to Liverpool Hospital, one by road ambulance and one by helicopter.
Another two of the men were taken to St George Hospital, and another two were taken to Wollongong Hospital.
Details of a seventh patient are at this stage unknown.
